The minimum qualification standards that are applied by our company in considering an applicant for an Independent Contractor position include the following:

1. *Are you 23 years of age or older? Yes No

2. *Do you have Three (3) years verifiable experience in the past five (5) years in the operation of a tractor-trailer unit, over the road, with a motor carrier? (This experience to include a minimum of one year’s experience with the type of trailer the applicant will be operating.) Yes No

3. *Do you have 3 or fewer points based on the point scale below? Yes No

a. Preventable DOT recordable accident
b. Serious traffic violation per Section 383.51
c. Non serious traffic violation per Section 383.51
d. Preventable Non DOT recordable accident

4. *Do you have any preventable DOT recordable accidents or serious traffic violations per 49 CFR 383.51 in the 12 months prior to date of application? Yes No

5. *Do you have a commercial transportation-related felony, theft or larceny conviction within your lifetime that resulted in an active prison or jail sentence? All other non-transportation related convictions will be subject to review at the discretion of the company and/or its insurance company.

6. *Do you have a conviction for DUI, DWI OR OWI on your driving record within the previous five (5) years?

EQUIPMENT SECTION

Please read our Out of Service Policy: If you are placed Out of Service during a roadside inspection, we will place you on probation for 6 months. If the OOS violation was for defective equipment, you will be instructed to go to a garage of our choice for an inspection within 48 hours. If the OOS violation was for you as a driver, you must come to Pittsburgh, PA within 30 days for additional training.

While on probation, if you have another OOS violation, your lease will be cancelled.

If any DOT inspection includes 3 or more separate OOS violations, your lease will be cancelled immediately.

7. *Do you have more than three (3) Out-of-Service violations on roadside inspections within two (2) years of application date? Yes No

8. The company, at its sole discretion, may decline an application due to any one or combination of the results received during the investigation of the applicants work history.

	*Have you had any cargo claims within the last 3 years? Yes No

	- If so, with which carriers?

	- Year of Tractor (Maximum age is 15 years) - Year of Trailer (Maximum age is 15 years)

	- Do you need a rental trailer? Yes No

	- If so, do you have your own chains, binders, and tarps? Yes No

	- We require a $300 deposit for rental trailers. Will you be able to bring $300 cash or money order to the orientation to obtain a rental trailer, if you need one? Yes No

As part of the application process, we will research your DOT Out of Service record and accident history with the DOT and/ or Federal Motor Carrier Safety Association.

NOTE- BEFORE WE WILL LEASE ANY TRUCKS OR TRAILERS, WE MUST HAVE AN INSPECTION FROM A TRUCK DEALERSHIP OR NATIONAL TRUCK STOP CHAIN, ON OUR FORM, WITHIN THE PRIOR 30 DAYS. WE WILL NOT ACCEPT INSPECTIONS FROM ANY OTHER GARAGES .
